ACOs provide coordinated, team-based, whole-person care to their patients, leading to better health outcomes, quality of life, and patient experiences. Use this form to share patient success stories. NAACOS will be highlighting key stories to help communicate the positive impacts of ACOs. We will follow-up with you directly prior to using your stories.

Briefly summarize your ACO patient success story below, including highlights of any data or key outcomes. Please include any relevant information related to challenges faced by the patient, solutions the ACO was able to provide that would not have been available in a traditional fee-for-service care setting, and positive impacts on health outcomes, quality of life, and/or patient/caregiver experiences. NAACOS will follow-up with you for additional details prior to using the story, which could be used to support data on how ACOs improve quality of care and patient experiences, to highlight the positive impacts of ACOs and value-based care for policymakers, or to communicate the benefits of receiving care from an ACO.
